Position Description
POSITION SUMMARY:
The Aging Services Social Worker is responsible for providing social services to AgeWell Cincinnati clients and K’vod Connect participants in need of telephonic or face-to-face supportive services. This may include in-home visits and follow-up, as well as ongoing care management and collaboration inter-departmentally within Jewish Family Service. The social worker helps clients/participants by assessing their situation; setting goals; and coordinating required services. The social worker also serves as an experienced and trusted advocate for the client/participant.
In addition, this social worker would have the opportunity to be a part of the AgeWell Cincinnati Concierge team, which offers additional compensation for private pay care management.
ESSENTIAL FUNCTIONS:
- Determines nature of client’s situation by meeting client, preferably in their home; assess client’s needs by gathering medical, psychological, emotional, and social information.
- Call handling of the AgeWell Cincinnati and JFS Helpline phone. Responds to inquiries via phone and email.
- Establishes a written care plan to improve the client’s well-being and executes that plan.
- Obtains assistance for client by referring him/her to community resources; arranging for appointments; and establishing rapport with our providers and other agencies.
- Acts as a liaison to families at a distance.
- Responds to clients in crisis situations.
- Maintains regular contact/interaction with client, family/caregiver, and other parties in order to coordinate services and monitor home situation.
- Enters client notes to track client’s progress, including outcome measurements. Completes and maintains all necessary documentation in Salesforce and ETO (software).
- Meets with walk-ins in our office.
- Provides follow-up telephone calls, in-home visits, and on-site visits (nursing homes, facilities, hospitals, etc.) as needed.
- Participates in assuring compliance to all in-house, local, state and federal regulations.
- Participates in in-services, local conferences and workshops.
- Thoroughly documents caller information in Salesforce for accurate tracking and analysis.
- Develops and maintains positive customer relations and coordinates with various functions within the agency to ensure customer requests and questions are handled in a timely manner.
- Participates in manning our information booth at sponsored events and programs.
- Performs other relevant duties as necessary.
- Pay range $24-29/hour
Position Requirements
POSITION QUALIFICATIONS:
- LSW required
- Master’s degree preferred
- Five years of work experience with older adults preferred
- Must have excellent customer relationship skills
- Able to work constructively as member of a departmental and operations team
- Strong technical skills (phone, computer, and software programs)
- Must possess professional work ethic and a team player
- Excellent verbal/written communication skills and able to communicate effectively across all levels of the organization and partner agencies.
Physical Requirements
This position requires walking, standing, and sitting. Position requires the use of a car to make home visits to clients in Hamilton, Butler, Warren, and Clermont Counties.
